Being hit by a shoe is actually the biggest insult and way to shame someone in middle eastern culture. Remember when the Sadamm statue fell and all the people chased it around the streets hitting it with their shoes? It was to show ultimate disrespect to him.
Which actually brings up a better question; if you insult someone and it doesn't culturally translate, did you actually insult them? I mean did you not fail in what you set out to do? Of course if Bush did not happen to have the reflexes of Reggie Bush and the shoe would have planted itself right in his nose then I think we could count the insult received regardless of culture.
